This is a simple salad that, once assembled, can be added to several meals. I like to have taco seasoning on hand to season a large batch of ground meat. A variety of healthy meals can be whipped up quickly. Consider putting meat and bean salad into a soft taco or use it as a dressing layer over lettuce.

According to this chart a cup of cooked black beans has over 19 grams of fiber that’s a majority of a person’s recommended daily intake.

Special thanks to Diabetes Bootcamp for providing the recipe.

Ingredients

2-4 cans of black beans

1 red onion

1 bunch cilantro (reserve 2 or 3 small bunches of leaves)

Juice of 3-4 limes (reserve 1-2 teaspoons)

1 jalapeño pepper

1 mango

1 avocado (optional)

3 T olive oil

Directions

Drain beans and put into serving bowl.

Dice ½ of the red onion; chop the leaves of the cilantro. Remove seeds from jalapeño (use disposable gloves), and then dice finely. Add onion, cilantro, jalapeño to beans and mix.

If using avocado, slice thinly then dice, squeezing a little lemon or lime juice over to keep from turning brown. Do the same for the mango, reserving each separately to add when ready to serve.

Mix in olive oil, lime juice (reserve a teaspoon or two for final preparation). Refrigerate to marinate flavors.

When ready to serve, add in mango and a little cilantro.

This is a unique creation that wins over many who say they “hate” polenta or grits and has become a favorite within my circle of family and friends. Upon hearing the amount of parsley, one foodie friend said, “have a little polenta with your parsley, Deb.”

I serve it as part of the second course (cooking meats, fish and tofu) of a fondue party–we warm it on the grill or fry it. It can be sliced toasted or grilled alone or with many toppings–treat it like a pizza crust. Diced it makes a unique crouton for salads.

Ingredients

1 cup course ground polenta (corn meal)

2 cups skim milk

2 cups water

1 cup Italian flat leaf parsley

2 tbs butter

1 tsp salt

Directions

Separate parsley leaves from stems. Put leaves in small food processor with salt and pulse then add cold butter and pulse again until little it forms little pills. Set aside.

Bring milk and water to a low boil then whisk in polenta; stir until all liquid has been absorbed.

Add parsley, salt, butter mixture until evenly distributed.

Pour out into loaf pan and put into refrigerator.

When chilled it will be very firm. Warm slices in a toaster oven or on a grill; excellent cubed and fried until golden brown.

Read story here

Scientists have developed a new device that works like a ‘rear-view mirror’ for the surgeon during bowel examinations, helping to detect 25 per cent more abnormalities.

Bowel cancer is Britain’s third most common cancer, with 40,000 new diagnoses a year.

The disease also claims 16,000 lives a year, making it the second most common cause of cancer death.

The Third Eye Retroscope is used along with a standard colonoscope to improve detection

That’s because, although the disease is treatable if detected early, 90 per cent of patients are diagnosed once the cancer is advanced, often because they are too embarrassed to seek medical help.

Symptoms include abdominal pain, diarrhoea, blood in the stools and unexpected weight loss.

To look for the early warning signs of bowel cancer, thousands of patients a year undergo a colonoscopy.

This 30-minute out-patient procedure, often carried out under sedation, involves a colonoscope — a thin, bendy tube with a video camera and light on the end of it — being inserted into the bowel.

As the device is withdrawn from the body, the camera relays video images of the inside of the bowel back to the doctor to check for abnormal growths, called adenomatous polyps or adenomas — these can then be removed before they become cancerous.

Research published in the New England Journal of Medicine earlier this year revealed that removing adenomas reduces deaths from bowel cancer by 53 per cent.

However, a number of studies published over the past 15 years have shown that traditional colonoscopies miss between 21 and 24 per cent of adenomas, and 12 per cent of large adenomas 1cm or more in size which are at greatest risk of becoming cancerous.

In two-thirds of cases, this is because they are hidden behind folds in the wall of the bowel.

In other cases, adenomas may be missed because the bowel has not been emptied properly before the procedure.

Now U.S. scientists have developed a new device, the Third Eye Retroscope, which is used along with a standard colonoscope to improve detection.

The Third Eye provides an additional video camera which gives a rear-facing view to reveal the areas behind folds that are hidden from the front-facing view of the colonoscope.

The rear-view camera is contained on a J-shaped attachment fixed to the tip of the colonoscope.

Studies show having that extra view significantly improves detection rates — especially among patients at high risk of bowel cancer (a family history of the disease, poor diet and increasing age are all risk factors).

An international study of 395 patients, published in the World Journal Of Gastroenterology earlier this year, revealed the Third Eye colonoscopy identifies 40 per cent more adenomas in people at high risk of bowel cancer, and 23 per cent more in the general population.

No large adenomas were missed using the new device.

For patients, the experience is exactly the same as a traditional colonoscopy.

Using the Third Eye does not add any time to the procedure, and preparation for it is exactly the same. However, doctors need additional training to teach them how to look at two images simultaneously.

The manufacturer says it takes around 15 to 20 cases for doctors to perfect the technique.

The disposable device, approved by the U.S. Food and Drug Administration in 2009 and which costs around £200, is currently available only in America, but the manufacturers plan to bring it to Britain within the next couple of years.

Commenting on the new device, consultant surgeon Tan Arulampalam, from Beating Bowel Cancer’s medical board, says: “A colonoscopy is one of the best ways to see whether there are any abnormal growths in the bowel. Initial studies into this new technique reveal that it could detect these polyps better because more of the bowel can be seen during the examination. Anything that improves early detection rates is a positive step forward. However, it isn’t widely available yet in the UK, so we recommend anyone worried about the symptoms of bowel cancer should visit their GP.”

“THIRD EYE CAMERA”:

The “third eye” is an improvement on traditional colonoscopy cameras because it’s other tip comes through the catheter revealing a backward facing camera with an LED light, whereas the previous colonoscopy camera had only a forward facing camera. A recent study revealed that the new “third eye” camera detects 41% more pre-cancerous polyps for people who are at a high risk of colon cancer and 23% more in people who are at not at risk.

In her recent blog post Ms. Bebinger poses several excellent questions, and we are answering them.

Here’s our perspective on questions related to safety and cost:

“What’s the doc’s error or complication rate per 1,000 patients? I do not want to see blood afterward, although if the doc finds and snips a polyp, I suppose I will.”

Colonoscopy is generally considered a safe procedure, and the benefits of early detection greatly outweigh the risks for most patients. Your individual health should be considered for any medical procedure.

Physicians who have more experience and do an adequate number of exams are more likely to be good at removing polyps of different shapes and sizes – big ones, flat ones, depressed ones and clusters [read more here about polyp morphology]. However, it’s like the Kenny Rogers lyric, “you gotta know when to hold ‘em, know when to fold ‘em.” Experienced endoscopists know when they need to “walk away” from what appears to be a very difficult removal and refer the patient to someone with even greater expertise.

And, yes, if you have polyps removed, you might see a small amount of blood. Follow the after-care instructions that you’re given just like you did for the bowel prep.

“One site says I should ask about the procedures for disinfecting equipment. Really? Isn’t flawless disinfection standard procedure?”

This issue received attention recently because there was a 2012 lawsuit in which a veteran won damages after contracting Hepatitis C from a dirty colonoscope.

Mistakes can occasionally happen, and infection in healthcare facilities is a complex problem that many are trying to solve. No matter what medical procedure you have, risks of infection are always a possibility, so you should consider your own health status when assessing your risks.

A high-definition colonoscope is a very expensive piece of medical equipment, and there are very specific protocols for cleaning, servicing and maintaining them in good working order. Hospitals and ambulatory surgery centers (ASC) have to be certified by local, state and national organizations. Following the manufacturer’s sterilization practices is one of many standard operating procedures (SOPs) in place for patient safety.

“And finally, I’ll ask how much each doc charges. I’m supposed to be looking for the best test at the best price.”

There are lots of variables that can affect the cost of a colonoscopy, and the most expensive exam isn’t necessarily the best. If you do a little research, you may be able to save some money, but be certain that you’re not sacrificing quality for that lower cost. But most importantly, don’t let the expense stop you from getting this life-saving test “behind you” (pun intended)!

You’ll likely spend more on car repair and veterinary bills each year than you’ll pay once every few years for this important test.

“Shopping for a colonoscopy could take weeks – not including the procrastination factor.”

We urge you to get through this task.
In her recent blog post Ms. Bebinger poses several excellent questions, and we are answering them.

Here’s our perspective on questions about quality measures:

“What’s the doctor’s detection rate? One medical society (the American Society for Gastrointestinal Endoscopy) says a doctor should find a polyp in 25% of men and 15% of women (why the difference?), but I know that some physician groups around Boston say the average is 40-50% among docs who really look for polyps.”

The expectation is higher for men because they tend to form more adenomas, the pre-cancerous polyps that are of most concern, but there’s no clear answer for why there is a difference. The guidelines of 25% for men and 15% for women are minimums, and most experienced physicians will find adenomas in many more patients. Bottom line (pun intended): more studies need to be performed specifically looking at gender differences to better understand the colorectal cancer disease process.

Next let’s briefly discuss why polyps get missed. The colon is not a straight smooth tube. It’s 6 feet long and must bend in many places to fit in the abdomen. Also, there are many folds in the wall of the colon that make it nearly impossible to view the entire lining with a colonoscope, which can look in only one direction – straight out of its tip. A pre-cancerous adenoma has no moral obligation to grow in an easy-to-see location – they often hide behind folds and turns. In fact, standard colonoscopy misses about 24% of adenomas, and 2/3 of those missed adenomas are hidden behind folds.

The Third Eye Retroscope is a device that can provide doctors with a second, “backward” view that allows them to see more of the polyps behind folds and turns. Third Eye colonoscopy studies indicate an increased adenoma detection rate of 23% in standard patient populations; however the adenoma detection rate increased to 40% for patients at above average risk of colorectal cancer. To read more about your risk profile for colorectal cancer read: “When Being Average is Just Fine Thanks.” To learn more about the importance of your screening test read High Stakes: Your First Colonoscopy.

“How much time does the doctor spend, on average, on the test? I think more is better, is that right?”

Physicians who rush through the exam too quickly are much more likely to miss adenomas, and most experts recommend spending at least 6-10 minutes observing the lining of the colon during withdrawal of the colonoscope. However, the duration of the exam is actually less important the quality of the examination technique. Conscientious physicians will spend much of that time flattening folds with the tip of the colonoscope in an attempt to see behind folds. Growing numbers of physicians are using a Third Eye Retroscope so they can get a more complete view of the areas behind those folds.

“Does the doctor always get to the end of the colon? OK, I may not be able to ask this. One website says I should ask the doctor to take a picture so that I know they got to the end – but this is too gross and how would I know what to look for anyway?”

Another anatomy lesson will provide all the confidence you need to understand the pictures doctors will show you to prove they got to the end of the road (technically the start of your colon, called the “cecum”). There are two major landmarks that help to identify the cecum. One is the ileocecal valve, where the small intestine empties into the colon. The other landmark is the appendiceal orifice – a small dimple where the appendix attaches to the cecum. Colonoscopists’ success rates for reaching the cecum should be in the high 90% range, and they should be really proud to tell you this number. It’s their version of putting a flag on Mount Everest.

In her recent blog post Ms. Bebinger poses several excellent questions, and we are answering them.

Here’s our perspective on questions related to the patients role and the physicians experience:

“About the prep – what will give me the best cleaning out with the least discomfort? There’s no point in going through all this if the doc can’t see what s/he is doing.”

We couldn’t agree more – the most important thing a patient can do, besides making the appointment, is being fully committed to the bowel preparation. There are many considerations when choosing the best bowel prep for your situation.

Every “body” is different, so different laxative formulations will work with different degrees of effectiveness. If you know a particular laxative has worked (or not) for you in the past, discuss that with your doctor.

Some pills and solutions are available over-the-counter (OTC) and some only by prescription (Rx).

Many doctors recommend a “split-prep” protocol, which has been shown to result in a much cleaner colon and a better quality of examination. A split-prep involves taking half the solution the evening before and half the morning of the procedure, and many patients have found this to be more tolerable than past protocols.
Hot Tip: ask doctors if they’ve ever completed a bowel prep themselves? The ones that have done it tend to be more sympathetic to their patients experience.

Certain medical conditions (e.g., diabetes) can narrow the options, and the protocol needs to be followed carefully.

There is a “low-residue” diet that can be followed the day leading up to the actual bowel cleansing that has been found to be acceptable by patients.
Fun fact: Ironically “low residue” foods are low in fiber (e.g., Wonder Bread). Personally, I gave them up a long time ago in service to better health, so it would be fun to eat them on “doctor’s orders” for a day.

“How many colonoscopies does the doc do each year? The avg., according to the ASGE, is 750. Again, more is better.”

Generally we agree that more is better, since experience counts. However, you may also want to be sure that they aren’t doing so many screening exams in a day that they don’t have time to do a thorough job.

“Shopping for a colonoscopy could take weeks – not including the procrastination factor.”

We urge you to get through this task. Once scheduled, there’s even an app to help you get through the bowel prep process: “Colonoscopy Prep Assistant”—it’s available for the iPhone and Android platforms.

Ms Bebinger: Let us know when you’ve completed your colonoscopy (via blog comment or Contact Us form) and we’ll send you a Good For You certificate of achievement!
